A complete blood count measures the cells in your blood — red cells, white cells, and platelets — and for most healthy women it is a normal, reassuring result. It can flag anemia (low oxygen-carrying capacity), infection, and some blood disorders: the World Health Organization estimated in 2019 that about 30 percent of women aged 15–49 worldwide live with anemia, and a CBC is the test that usually finds it first.

What does a complete blood count actually measure?
A CBC, short for complete blood count, is a panel of measurements produced by an automated analyzer that counts and sizes the three main types of blood cells. Most lab reports include three groups of values, and each group tells a different part of the story.
- Red blood cells. The report shows hemoglobin (the oxygen-carrying protein), hematocrit (the share of blood volume made up of red cells), red cell count, and mean corpuscular volume (MCV), which describes whether red cells are small, normal, or large. The CDC notes that hemoglobin and hematocrit are the measurements behind most anemia diagnoses.
- White blood cells. The white cell count and its five-part differential (neutrophils, lymphocytes, monocytes, eosinophils, basophils) reflect the immune system's activity. A high count often accompanies infection; a low count can follow some viral illnesses or certain medications.
- Platelets. These are the cell fragments that help blood clot. The count matters before surgery and when unusual bruising or bleeding appears.
Many reports also include RDW (red cell distribution width), which describes how much red cells vary in size — an early clue in iron deficiency.
When do clinicians order a CBC for women?
Clinicians order a CBC for routine evaluation when symptoms such as fatigue, dizziness, or heavy menstrual bleeding suggest anemia; when an infection is suspected; before surgery; during pregnancy, when anemia screening is standard; and to monitor medications that can suppress blood cell production, per NIH and ACOG guidance. It is inexpensive, fast, and low-risk, which is why it appears so often. A normal CBC is genuinely useful information — it makes several common explanations for symptoms less likely, though it does not rule everything out.
What can a CBC tell you about anemia?
The CBC is the starting point for almost every anemia workup. If hemoglobin falls below the reference range, the MCV helps sort the likely causes: small red cells (low MCV) point toward iron deficiency, often driven by heavy periods in premenopausal women, while large red cells (high MCV) raise questions about vitamin B12 or folate deficiency, thyroid disease, or alcohol use. Per WHO estimates published in 2019, roughly three in ten women of reproductive age worldwide are anemic, and iron deficiency is the most common single cause.
What the CBC alone cannot do is say why hemoglobin is low. Iron studies (ferritin), B12, folate, and sometimes tests for celiac disease or heavy menstrual blood loss are needed to find the mechanism. A 2023 review in JAMA noted that ferritin is the most useful single test for distinguishing iron deficiency anemia from other types — a CBC raises the question, follow-up tests answer it.
Can a CBC detect cancer or serious disease?
Sometimes it raises flags, but it is not a cancer screening test. A CBC can reveal leukemias and some other blood disorders, and the American Cancer Society notes that blood counts are one route by which such conditions are first detected. Far more often, mild abnormalities — a white count slightly above range during a cold, hemoglobin a point below range — reflect ordinary, temporary states. Blood cancer is rare, and single mildly abnormal results in people without symptoms usually warrant observation or a repeat test rather than alarm. No medical organization recommends a CBC as a routine cancer screen in healthy adults without symptoms.

What a normal CBC does not rule out
A normal CBC is reassuring but narrow. It does not measure hormones, blood sugar, cholesterol, kidney or thyroid function, vitamins other than what follow-up tests cover, or inflammation in a sensitive way. It cannot detect endometriosis, polycystic ovary syndrome, osteoporosis, heart disease risk, or most early cancers. Feeling unwell with a normal CBC is a real and common experience — the explanation usually lies outside this panel, and the search continues with other tests, not with dismissal.
Reference ranges also have limits. Labs define "normal" from population statistics, so a value slightly outside the range may be normal for you, and a value inside the range can still be abnormal for your situation. Trends over time often matter more than a single snapshot, which is why clinicians compare new results with older ones.
How should you read your own CBC report?
A reasonable approach is to look at three things and leave the rest to your clinician:
- Hemoglobin. If it is near or below the lower limit and you have symptoms such as fatigue, breathlessness on stairs, or a racing heart, ask whether iron studies are warranted — especially if your periods are heavy.
- Whether the abnormality is mild or marked. Values just outside the range after an illness often normalize on repeat testing; values far outside it need prompt attention.
- Patterns, not single lines. Low hemoglobin with low MCV and high RDW tells a different story than any one of those alone.
When to talk to a clinician
Contact a clinician if you have symptoms of anemia — persistent fatigue, breathlessness with ordinary activity, palpitations, or pallor — or if you soak through a pad or tampon every hour for several hours in a row, which ACOG's guidance defines as heavy bleeding that warrants evaluation. Seek care promptly for unexplained bruising, frequent nosebleeds, fevers with an abnormal white count, or any result your lab or clinician has flagged as urgent. And if a result sits outside the range and you feel fine, a short conversation about whether and when to retest is usually enough — most mild abnormalities do not require urgent action.

Do I need to fast before a CBC? No. A CBC does not require fasting, per NIH MedlinePlus guidance, and you can eat and drink normally beforehand. Fasting is only needed when the same blood draw includes tests such as glucose or cholesterol. Hydration is still worthwhile, since dehydration can slightly concentrate the blood and shift some values.
Why do reference ranges differ between labs? Each laboratory calculates its ranges from the population and instruments it uses, so the numbers at the edges vary. A hemoglobin of 11.9 g/dL may be flagged at one lab and not another. This is why clinicians interpret results against the specific lab's range and against your own prior results rather than against a universal standard.
Can heavy periods really change my CBC? Yes. Regular heavy menstrual bleeding is one of the most common causes of iron deficiency in premenopausal women, and ACOG's 2022 guidance recommends clinicians consider iron deficiency in patients with heavy bleeding even when anemia has not yet developed. Over time, ongoing loss can outpace dietary iron intake and first appear as a low hemoglobin on a routine CBC.
How often should a healthy woman have a CBC? There is no universal schedule. The USPSTF and other bodies have not endorsed routine annual CBC screening for healthy adults without symptoms. Clinicians typically order one when symptoms suggest a blood-related issue, during pregnancy, before procedures, or to monitor a medication — the reason for the test drives its timing, not the calendar.
Is a low white blood cell count dangerous? Usually not on its own. Mildly low counts are common after viral infections and can be a normal ethnic and individual variation, per NIH MedlinePlus. Counts that keep falling, or that come with fevers, mouth sores, or recurrent infections, need evaluation. A single mildly low result with no symptoms typically leads to a repeat test in a few weeks rather than treatment.
